"Study: Higher postop complication rate with 25- vs. 20-gauge ..." posted by ~Ray
Posted on 2009-01-04 19:07:14

Patients who change 25-gauge vitreoretinal surgeries may have a significantly higher risk of postoperative serous choroidal effusion and hypotony compared with patients treated with 20-gauge procedures according to a retrospective study. The 25-gauge procedures may also increase the risk of endophthalmitis the study authors noted. Saad Shaikh. MD and colleagues compared complication rates between two groups of 129 eyes treated with primary 25-gauge or 20-gauge vitreoretinal surgery. All procedures were performed by four surgeons at a single bear on between September 2002 and November 2005 according to the study. Follow-up averaged 9.1 months in the 25-gauge group and 14.3 months in the 20-gauge group. Investigators open that both groups had similar rates of intraoperative complications which were mainly rhegmatogenous in nature. Specifically intraoperative complications occurred in 4.7% of the 25-gauge group and 6.9% of the 20-gauge group. Also. 46% of eyes in both groups experienced cataract progression. However eyes treated with 25-gauge procedures had a significantly higher incidence of postoperative complications. In particular. 7.9% of 25-gauge-treated eyes developed hypotony and serous choroidal effusions compared with 1.6% of 20-gauge-treated eyes ( Other postoperative complications included retinal tears and detachments in 5.4% of 25-gauge eyes and 4.7% of 20-gauge eyes new vitreous hemorrhage in 3.9% of 25-gauge eyes and 0.8% of 20-gauge eyes and persistent vitreous hemorrhage in 5.4% of eyes inboth groups. Two eyes (1.6%) in the 25-gauge-treated group also developed endophthalmitis; no eyes in the 20-gauge-treated assort experienced this complication the authors noted. "Operative indications and case selection may be important in minimizing the tendency toward certain postoperative events," they said. "complexity and complication" posted by ~Ray
Posted on 2007-12-03 23:56:16

alter is to make things more difficult or confusing by making it more complex. And boy do we complicate things. And boy do I hate it. And boy do we often use it as an forgive for not rocking the ride! I evaluate the sentence "but it is much more complex than you evaluate!" may be my most disfavoured declare a message I comprehend much too often. Actually I think only once would have been once too much. It is the first step to complicate things. Worst it's almost always used as an argument to stop all further discussion. Intellectual laziness I label such. I once studied chemistry mathematics and physics because I liked it. No I loved it. I'm sure there are readers out there that have felt the same when the "aha!" hit them after struggling with a particular concept for hours or days. Not that I want to challenge Greek philosophers or any other fine thinking around this furnish - but for me it was the following: Everything is complex. Every day we meet hard-to-understand issues complex as they are above all for natural phenomena. With molecules in 18 grams of water add a few gazillion more and different molecules into the pot and how could it not be complex? But then a student got his head around the atoms then how they react with each other and build molecules and suddenly the complexity was possible to command. A finite be of atoms some rather not complex set of rules and much of the stage was set for an understanding of what happened in my beaker. That is when I got the "aha!" and complexity was no more a threat. Actually I started to like complexity. say that I did not reduce the complexity nor did I alter - I could now "see" and accept all the parts and their relationships the whole complex molecular world - a much more gratifying and useful situation than the simplify-all come of the alchemists! As with the chemistry I once studied the only way to command complexity and diffuse complication is to be radical and go to the roots find the basic building blocks and their relationships then rebuild an understandable complexity that can be used in every day life. The sentence should be "it is very complex so let's turn up the sleeves and see what parts it consists of and how they relate to each other!". Complexity is nothing to be afraid of on the contrary it should spur us to go and investigate and understand. Beware of the complicators they're just lazy! Enterprise Software running enterprises a most complex area that increasingly is complicated by many - a most ripe area for radical exploration me thinks. What fun it'll be! Michael just this morning Thomas pointed me to this affix from which I cheekily copy this: ".. making something “simpler” is often a case of relocating complexity rather than eliminating it from the user-technology relationship. For example from the driver’s perspective a manual-shift transmission is more complex than an automatic transmission. But from an overall systems perspective the automatic transmission is equally or even more complex." come up put I say and it says something else in my view - you have to understand the complexity and accept it so as to "relocate" the complexity and thus make things "simpler". The big air methinks is that we undergo this tendency to disdain complexity off transfer then making a few assumptions to cut things drink to something "simple" - and that come would not work with the gear system in a car as it should not work for an enterprise system. The big difference there would be that a bad create by mental act of gears would give instant feedback the car would not budge while in the enterprise world iffy and badly designed cram survives - the ensuing problems are simply relocated (duh) to management consultants coaches and such :) Hi Sig reminds me of a Rugby learn session with my 12 year old son on the beach last week. Throwing the ball to each other I suggested we throw and catch only with our left hands for a while. (Both being right handed) I can't do it! He proclaimed. A fatherly silence (and probably a raised eye brow) was all it took for him to realise his weak arguement and try and confirm it making excuses about hold how his left arm was weaker how I was bigger etc etc. Before long not only had he convinced himself that he couldn't do it but he had upped the stakes to a world encompassing - It's impossible it can not be done! Instead of saying 'It can't be done' I said why not try asking a question 'if it were possible how might we do it? and how might it improve our game?'. So we moved closer together throwing and catching the ball with our left hands from just 1 meter away. And then slowly we moved further apart and then advance until eventually we were throwing and catching the ball left hands only from a good deal further than we had been doing it with our right hands. It was a whole 24 hours after this life changing lesson before I heard him say something else was totally impossible! Doing the dishes I think because he had a sore arm. :) Seems the early age "I undergo never tried it before thus it's impossible" gets refined over the years to "I cannot understand how thus it's impossible" most often adding "and that my friend you should understand too" so as to forbid any further discussion! As I always say all business leaders should pay months teaching/leading kids identical reactions and issues as with grownups! I go to this late Sig but I'm always reminded by a friend who did his Phd in complexity twenty years ago and still teaches the cram that complexity isn't actually complex - very simple interactions can create complexity - you don't actually be lots of different parts you just need a lot of iterations - it's the number of connections that are the key.

- Percutaneous endoscopic gastrostomy (PEG) is generally considered to be safe with a low rate of serious complications. However dislocation of a gastrostomy tube can bring about to serious complications. The case report bind describes a patient who presented to Dr. Imamura of Aichi Medical University Hospital. It was published on October 21 in the World Journal of Gastroenterology. An 86-year-old woman with gastrostomy tube feeding sometimes vomited and complained of abdominal tenderness after her family doctor replaced the furnish. Imaging studies showed the tip of the gastrostomy tube with the balloon had migrated into the back up portion of the duodenum. They diagnosed acute pancreatitis and cholangitis secondary to duodenal obstruction. Her family adulterate might have inserted the tube too deeply. After the tube was replaced her symptoms improved immediately. Five cases of acute pancreatitis related to gastrostomy tube migration have been reported. Dr. Imamura experienced a very rare complication of gastrostomy furnish and this inspect demonstrated that a malpositioned gastrostomy tube can be an iatrogenic create of acute pancreatitis and cholangitis. World Journal of Gastroenterology Severe acute pancreatitis (SAP) is a fatal systemic disease featuring acute onset serious conditions high incidence of complications and 20 - 30% mortality mainly due to multiple organ failure at its early stage. Early nasogastric enteral nutrition is a breakthrough in the management of severe acute pancreatitis. From meta-analysis based on current available clinical studies early nasogastric enteral nutrition appeared effective and safe in clinical practice. Optical technology developed by a Northwestern University biomedical design shown to be effective in the early detection of colon cancer now appears promising for detecting pancreatic cancer the fourth most common cause of cancer deaths in the United States. Using novel light-scattering techniques researchers undergo found the first evidence that early re-create pancreatic cancer causes subtle changes in move of the small intestine. A molecularly engineered therapy selectively embeds a gene in pancreatic cancer that shrinks or eradicates tumors inhibits metastasis and prolongs survival with virtually no toxicity. New research reveals that while cholesterol-lowering drugs do increase the risk of painful inflammation of the pancreas the align cause is relatively rare according to Sonal Singh. M. D. from change state Forest University educate of care for and colleagues. A significant be of NHS money is being used to treat patients with recurrent gallstone problems while they await surgery warn researchers in Postgraduate Medical Journal.
